Job Summary
Assess patients, monitor status, and communicate changes to providers while maintaining accurate care plans and EMR documentation. Support patient rooming, screenings, intake, scheduling, and care coordination by responding to phone calls, MyChart messages, and provider in-baskets. Assist patients with navigating appointments, referrals, and follow-up care, collaborating with the team to support Chronic Care Management and quality initiatives. Monitor patient needs, coordinate services, and help identify opportunities to improve engagement and continuity of care. This part-time role offers a mix of clinical care and patient engagement within Lakewood Health System's rural healthcare network.
Required Qualifications
- Active Minnesota LPN licensure or current CMA/AAMA certification
- Current BLS Healthcare Provider certification
- Strong clinical and patient-care skills within scope of practice
- Proficiency in EMR documentation and workflow management
- Ability to prioritize tasks, problem-solve, and work independently
- Strong organizational skills and attention to detail in documentation and reporting
- Effective communication and collaboration with patients, providers, and healthcare teams
