LPN - Clinical Liaison/Marketing

Determine patient eligibility for hospice care by reviewing medical records and assessing patients upon admission. Communicate with families to explain care options while developing community relationships on behalf of the facility. Schedule speaking engagements at community events and collaborate with the marketing and sales team to promote the facility's strengths and services. Interact with hospital staff across different departments to facilitate care coordination.
