LPN Clinical Coordinator Home Health

LPN Clinical Coordinator – Home Health coordinates patient care and clinical processes, including clinical documentation review, patient coordination, scheduling, and triage support. Works closely with clinicians, administrative staff, and external vendors to ensure timely, high-quality home health care, regulatory compliance, and efficient workflows. May provide coverage for the Director of Clinical Services in their absence and contribute to case management, quality improvement, and discharge workflows. Proven nursing knowledge, strong organizational and problem-solving skills, proficiency with home health regulations, documentation, and communication are essential.
