Job Summary

Reviews and prepares medical records, initiates follow-ups, and identifies patient needs to prepare orders for practitioner authorization. Administers treatments, immunizations, and medications as prescribed; performs point-of-care testing and provides laboratory and x-ray reports. Documents interventions on appropriate forms or online records using department and JCAHO standards. Manages all patient vaccine data, including updating the Hawaii Immunization Registry System, EHR, and external records. Stores and administers immunizations according to Health Plan and VFC guidelines, maintaining inventory and coordinating state liaison duties. Trains new and existing LPNs on emergency vaccine storage and department procedures. Follows discharge plans, processes referrals, and provides patient education while documenting responses. Schedules patients for routine and acute care, addresses care gaps, and maintains supply inventories.

Required Qualifications

  • Twelve (12) months (2080 hours) LPN experience
  • Hawaii State Board of Nursing approved curriculum for LPN licensure
  • Licensed Practical Nurse (Hawaii) required at hire
  • Basic Life Support from American Heart Association
  • Demonstrated knowledge of and skill in conflict resolution
  • Demonstrated knowledge of and skill in customer service
  • Demonstrated knowledge of and skill in interpersonal relations
  • Demonstrated knowledge of and skill in oral communication
  • Demonstrated knowledge of and skill in problem solving
  • Demonstrated knowledge of and skill in teamwork
  • Demonstrated knowledge of and skill in written communication
  • Talking to co-workers, customers, outside vendors, and on the telephone
  • Reading, writing, speaking and understanding English
  • Training/giving and receiving instructions
  • Mathematical ability
  • Attention to detail (e.g., organization, prioritization, proofing)
  • Concentration and alertness
  • OHS only: Certification in Spirometry, Hearing Conservation, & Substance Abuse Specimen Collection or acquire certification within one year of hire

Desired Qualifications

  • Acute care nursing experience
  • Demonstrated knowledge of and skill in word processing and spreadsheet PC applications
