LPN - Cliffside Park NJ

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under the direction of a Coordinator of Care RN or RN. Perform revisits to patients based on skilled need, implement care plans, and evaluate treatment outcomes. Accurately observe, measure, record, and report clinical data, including documenting signs of health decline. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning. Document visits electronically within 24-48 hours. Requires NY State LPN license, one year Med/surg experience, driver's license, and strong computer skills.
