Job Summary
Administer medications, monitor vital signs, and implement care plans for residents under RN supervision. Collaborate with the interdisciplinary healthcare team to develop individualized care plans and establish goals. Accurately document all care, medication administration, and condition changes while maintaining compliance with state regulations. Perform wound care, including dressing changes, and respond to medical emergencies by initiating CPR and following established protocols. Educate residents and families on disease management and self-care techniques. Work flexible hours, including weekends and holidays, while adhering to infection control protocols and safety standards.
Required Qualifications
- Graduation from an accredited practical nursing program
- current Licensed Practical Nurse (LPN) licensure
- Knowledge of nursing principles, practices, and procedures
- Strong clinical assessment skills
- ability to identify resident condition changes
- Proficiency in medication administration
- knowledge of common medications and their effects
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a healthcare team
- Attention to detail
- Physical stamina
- ability to perform tasks that may involve lifting, bending, and standing for extended periods
- Familiarity with electronic health record (EHR) systems
- Willingness to work flexible hours, including weekends and holidays
- Must be a supportive team member
