LPN Charge Nurse - All Shifts Available

Job Summary
Administer medications, monitor vital signs, and assist with treatments as prescribed by physicians or RNs. Develop and implement individualized care plans while collaborating with the interdisciplinary healthcare team to meet residents' physical and emotional needs. Accurately document care, medication administration, and condition changes in compliance with facility policies and state regulations. Perform wound care, including dressing changes, and respond quickly to medical emergencies following established protocols. Educate residents and families about medical conditions and self-care techniques while advocating for their rights and preferences. Maintain accurate medication records, monitor responses to treatments, and report adverse reactions promptly. Work flexible hours including weekends and holidays, and be available for emergency callbacks when necessary.
Required Qualifications
- Graduation from an accredited practical nursing program
- current Licensed Practical Nurse (LPN) licensure
- Knowledge of nursing principles, practices, and procedures
- Strong clinical assessment skills and the ability to identify resident condition changes
- Proficiency in medication administration and knowledge of common medications and their effects
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a healthcare team
- Attention to detail and accuracy in documentation
- Compassionate and empathetic demeanor
- Physical stamina and the ability to perform tasks that may involve lifting, bending, and standing for extended periods
- Familiarity with electronic health record (EHR) systems and healthcare technology
- Willingness to work flexible hours, including weekends and holidays, per the facility's needs
- Must be able to move intermittently throughout the workday
- Must be able to read, speak, and understandably write the English language
- Must be able to cope with the mental and emotional stress of the position
- Must function independently, and have flexibility, personal integrity, and the ability to work effectively with customers, residents, personnel, and support agencies
- Must be able to push, pull, move, and/or lift a minimum of 50 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet
- Must be a supportive team member, contribute to and be an example of teamwork and team concept
- Must possess the ability to make independent decisions when circumstances warrant such action
- Must possess the ability to deal tactfully with customers, personnel, residents, family members, visitors, government agencies/personnel, and the general public
- Must not pose a direct threat to the health or safety of other individuals in the workplace
- Must meet the general health requirements set forth by the policies of this company, which include a medical and physical examination if the position requires
