LPN Case Manager - Multiple locations and positions!!

LPN Case Manager coordinates health care and medical case management for clients, performing basic health assessments, monitoring vitals, and providing ongoing health support. Responsibilities include making referrals for medical, substance use, and psychiatric treatment, managing medications, collaborating with medical/psychiatric providers and hospitals, developing health-related education groups and one-on-one client education, participating in interdisciplinary team meetings and case conferences, responding to medical emergencies with first aid/CPR and Narcan when needed, and ensuring compliance with regulatory guidelines. The role requires an NYS LPN license, prefers a Bachelor’s Degree, and values experience with homelessness, mental illness, and substance use disorders, with CPR certification and MS Office proficiency; bilingual skills are a plus.
