Job Summary
Coordinate health care and medical case management for clients, perform basic health assessments, monitor vitals, manage referrals for medical/substance use/psychiatric treatment, document medication self-management, process physicians' orders, collaborate with medical/psychiatric providers and hospitals, develop health-related education groups and one-on-one education, participate in interdisciplinary meetings, respond to medical emergencies with CPR and Narcan as needed, ensure regulatory compliance, and support crisis response within BRC programs in Manhattan (full-time)
Required Qualifications
- NYS LPN license required
- Bachelor's Degree preferred
- Experience working with individuals experiencing homelessness preferred
- CPR training certification or willingness to complete certification training in CPR and opioid overdose prevention

- Vaccination preferred but not required
