Job Summary
LPN Case Manager for a 146-apartment housing program. Performs basic health assessments (blood pressure, weight, etc.), manages client health needs, makes referrals for medical, substance use, and psychiatric treatment, develops and facilitates health-related educational groups, participates in treatment and interdisciplinary team meetings, assists in goal development, handles chart documentation, and provides emergency first aid/CPR as needed.
Required Qualifications
- LPN license required.
- Experience in substance use/addiction, homelessness, psychiatric and/or medical nursing preferred.
- Basic computer literacy required.
- CPR training certification or willingness to complete training class in CPR required.
