LPN Case Manager - 13-12 Beach Channel Dr

Job Summary
Coordinate health care and medical-related case management for clients. Perform basic health assessments, monitor vitals (including blood pressure and weight), and provide ongoing health monitoring. Make referrals and follow-ups for medical, substance use, and psychiatric treatment; observe and document client self-management of medication; process physicians' orders and obtain prescriptions. Collaborate with medical and psychiatric providers and hospitals; develop and deliver health-related education through groups and one-on-one sessions. Participate in interdisciplinary team meetings and case conferences; respond to medical emergencies with emergency first aid/CPR and Narcan opioid overdose reversal when needed; address program crises as required. Ensure compliance with regulatory and agency guidelines. Related duties as assigned.
Required Qualifications
- NYS LPN license required
- Bachelor's Degree preferred
- Experience with homelessness preferred
- Proficiency in MS Office (Word, Excel)
- CPR certification or willingness to obtain CPR and opioid overdose prevention training
