LPN Case Manager - 127 West 25th Street

Coordinate the health care and medical-related case management for clients. Perform basic health assessments, monitor vitals (blood pressure and weight), provide ongoing health monitoring, and make referrals/follow-ups for medical, substance-use, and psychiatric treatment. Manage medication self-management documentation, process physicians’ orders, and obtain prescriptions. Collaborate with medical/psychiatric providers and hospitals; develop and facilitate health-related education for groups and individuals. Participate in interdisciplinary team meetings and case conferences; respond to medical emergencies with first aid/CPR and Narcan overdose reversal as needed. Ensure regulatory and agency guideline compliance; support crises response as required.
