LPN Case Coordinator

Coordinate patient care from intake through discharge by guiding patients, taking vital signs, and managing treatment plans with physicians, pharmacists, and therapists. Handle incoming calls, pull charts, coordinate pharmacy refills, and manage physical/occupational therapy re-certifications while addressing pre-authorization requirements. Maintain staff schedules, order supplies, and complete documentation in accordance with hospital policy. This remote position requires a licensed Practical Nurse in Washington with two years of clinical experience and Basic Life Support certification.
