Job Summary
Provide follow-up care to risk-stratified patients post-discharge from emergency rooms, inpatient venues, or readmissions, ensuring adequate medication education and addressing barriers to care including social determinants. Collaborate with patients, families, and care teams to manage medical conditions and behavioral health needs while coordinating transitions between home, hospital, rehab, and outpatient settings. Determine encounter frequency, conduct face-to-face visits when necessary, and develop individualized care plans for geriatric and chronic disease management. Make referrals for complex high-risk patients and complete designated self-chart audits. Work with visiting nurses, support groups, and community stakeholders to improve health outcomes.
Required Qualifications
- Current Vermont RN/LPN license
- CPR Certification
- Prior experience working in a nursing position
- Prior case management experience in a similar outpatient setting preferred

- Experience in using a variety of electronic medical record
- Ability to learn other systems
- Basic keyboarding skills
- Email communication
