Job Summary
Oversee care for patients transitioning from healthcare facilities by coordinating with Primary Care Providers, managing registries for high-risk patients, and arranging follow-up visits. Handle incoming phone calls to create orders, deliver test results, manage medication refills, and schedule specialty appointments. Perform clinical tasks including drawing medications, monitoring Ambulatory Blood Pressure and Continuous Glucose Monitoring devices, and ensuring HIPAA confidentiality. Work evenings, weekends, and holidays as part of a rotation schedule to meet patient needs.
Required Qualifications
- An active Licensed Practical Nurse license
- Basic Life Support (BLS) certification required
- Graduate of an accredited LPN program
- Active Pennsylvania Practical Nursing license
- BLS Certification
- Ability to work as part of a care team
- Excellent communication skills
- Commitment to process improvement and quality
- Evenings (one late night per week; 12PM-8PM)
- Weekends (either one Saturday or Sunday per month; when worked, employees will have a day off during the week)
- Holidays (one to two holidays per year)
Desired Qualifications
- Buy in and experience working in the EOS® model (strongly preferred)
- Mission-oriented; represents the enterprise in a professional manner while demonstrating organizational pride
- Previous experience in a health center setting a plus
- Experience with electronic health record, Medent a plus
