LPN Care Coordinator- Rural Health Transf.

LPN Care Coordinator for the Rural Health Transformation program conducts telephonic/virtual outreach, performs targeted assessments and coaching, administers medication reconciliation, supports implementation of care plans, collaborates with RN and multidisciplinary team, and facilitates access to HUSKY Health and community resources. Responsibilities include risk screening, patient education, documentation in electronic care management systems, participation in interdisciplinary rounds, and providing written/digital educational resources to support member learning. Requires strong organizational and communication skills, knowledge of HUSKY Health benefits, and an active LPN license.
