Job Summary
LPN Care Coordinator in Beverly, MA coordinates geriatric outpatient and home-based care within a PACE program, collaborating with Occupational Therapy and nursing teams to assess patient needs, update care plans, and support patient independence. The role is primarily clinic-based with occasional home visits, Monday–Friday 8:00 AM–4:00 PM, and carries no weekend or on-call duties. Requires an active MA LPN license and CPR, with a focus on geriatric care and interdisciplinary collaboration. Required Qualifications
