Job Summary
Build trusted relationships with residents and families to resolve concerns and support independent living within the Health & Wellness Navigation Program. Assist residents in transitioning across the continuum of care by coordinating services, communicating with stakeholders, and facilitating access to internal and external resources. Lead the Radar Meeting to strategize resident outcomes and campus flow while overseeing cross-functional post-acute support. Maintain emergency medical information, review incident reports for risk reduction, and partner with departments to implement health and wellness programming. Achieve bronze, silver, and gold certification standards while collaborating with Manor Care staff to manage admissions, discharges, and supportive services.
Required Qualifications
- Three to five years of experience working in the Senior Living Space, Independent Living, Assisted Living, and Memory Care or other health care setting
- Graduate of an accredited college with an LPN, or RN
- Bachelor's degree in social services, geriatrics, nursing, and or related senior living services preferred
- Strong interpersonal skills include the ability to motivate and encourage residents to achieve maximum independence and quality of life
- Ability to problem-solve, make ethical recommendations, define appropriate boundaries and be resolution oriented
- Excellent verbal and written communication skills for groups and individuals of different populations including persons with disabilities and Dementia
- Effective organizational skills including delegation, managing multiple priorities, time management
- Ability to collaborate with community management, internal and external resources/partners and communicate with residents, families, physicians, and staff
- Regular, reliable and punctual attendance
- This position requires the ability to perform the essential functions of the job with or without reasonable accommodation
- This position primarily uses a personal computer and general office equipment, including a printer, scanner, fax machine, copier, and telephone
- The employee must be able to sit, stand, walk, talk, listen, reach, bend, twist, and climb on an occasional to frequent basis as required by job duties
- This role may require occasional lifting, carrying, pushing, or pulling of up to 50 pounds with assistance
- The ability to perform sedentary work involving exertion of up to 10 pounds of force occasionally and/or up to 20 pounds of force occasionally to move objects
- The employee must be able to routinely read, interpret, and complete written documentation
- The employee must be able to observe resident actions, gestures, and facial expressions
- The employee must be able to recognize auditory call signals, telephone rings, soft voices, and workplace communication
- The employee must be able to identify odors relevant to resident care or environmental conditions
- Required personal protective equipment must be used as dictated by working conditions, policy, and applicable safety requirements
