Job Summary
Assess patients, manage medication administration, and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Reconcile prescribed medications, monitor clinical data, and document visits electronically within 24-48 hours. Conduct patient revisits based on skilled needs to oversee recovery processes from illness, injury, or surgery at home. Requires NY State LPN license, one year of med/surg experience, and a valid driver's license.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
