LPN - Bronx NY

Conduct advanced clinical home care services, including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled need, implement the plan of care, and evaluate treatment outcomes. Accurately observe, measure, record, and report clinical data related to patient health status, while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Collect and communicate signs of health decline immediately. Requires NY State LPN license, one year of med/surg experience, a driver's license, and good computer skills.
