Job Summary

Provide skilled nursing care to participants, including wound care, medication administration, and immunizations. Perform patient education on medications and treatments, conduct nursing follow-ups after ER visits or falls, and collect lab specimens with proper biohazard precautions. Document assessments and care coordination in the EMR, implement home care orders, and communicate with the Interdisciplinary Team, families, and providers using SBAR format. Requires Colorado licensure, one year of healthcare experience, and proficiency with electronic health records.

Required Qualifications

  • Graduate of an accredited Licensed Practical Nurse Program
  • Licensed as a professional in the State of Colorado
  • One year of health care experience in hospital, long-term or home health care setting
  • 1 year of experience with a frail or elderly population
  • Ability to perform phlebotomy functions
  • Knowledge of medical equipment and instruments
  • Excellent written and verbal communication skills
  • Demonstrated ability to interact clearly and effectively with both internal and external customers
  • Reliable transportation
  • Current automobile insurance
  • Efficiency with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records

Desired Qualifications

  • One year of health care experience in hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is preferred
