Job Summary
Deliver resident care through assessment, planning, intervention, and implementation while monitoring medication administration and updating service plans. Ensure resident safety by observing needs, communicating changes to the clinical team, and holding RA/CNA staff accountable to care plans. Provide education to families, coordinate shift-to-shift responsibilities, and maintain accurate EMR documentation and communication logs. Act as the primary point of contact for emergencies, notify families of hospitalizations, and report observations to physicians and therapists. Maintain operational readiness by ensuring supplies are available and responding to changing situations with flexibility.
Required Qualifications
- Current valid certification as a Registered Nurse or Licensed Practical Nurse in the State of Illinois
- Current CPR certification
- Must be able to push, pull, move, and/or lift a minimum of twenty-five (25) pounds
- Is subject to callback during emergency conditions and may be necessary to assist in the evacuation of residents
- Individual will be scheduled based on operational need
- Works beyond normal working hours and on weekends and holidays when necessary
- Sits, stands, bends, lifts, walks and climbs stairs, as needed to conduct nursing activities
- Must possess leadership ability and willingness to work harmoniously with and to direct other personnel
- Ability to communicate effectively both verbally and in written form with a diverse group of people
- Ability and willingness to effectively motivate and lead individuals and groups with diverse needs and abilities
- Ability and willingness to work harmoniously within a team environment departmentally
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public
- Must possess the ability to make independent decisions when circumstances warrant such action
- Must be able to maintain good personnel relations and employee moral
- Aptitude for detail work
- Basic computer literacy and familiarity with EMR or willingness to learn EMR system
- Must be able to read, write, speak, and understand the English language
- Must be able to cope with the mental and emotional stress of the position
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility
