Job Summary

Observe and document the physical, mental, and emotional needs of patients and families while delivering prescribed physical and psychological care. Consult with the Hospice Nurse regarding patient status and symptom management, then monitor and document family input and response to care. Maintain continuity of care through ongoing communication with families and health professionals, while ensuring adequate home care supplies and assisting with clinical billing. Participate in Interdisciplinary Team Meetings and assist with orientation of new hospice aides as assigned. Requires a valid LPN license, one year of clinical experience, and strong communication skills.

Required Qualifications

  • Associate's Degree in a related field
  • Hold a Valid and current LPN license within the state of operations
  • 1 year of clinical experience
  • Strong verbal and written communication skills
  • Able to work as part of an inter-disciplinary team

Desired Qualifications

  • Long Term Care/Home Care/Hospice experience preferred
