Job Summary
Provide direct clinical and nursing services as an LPN within two Zepf Center sites, focusing on medical case management and advocacy for clients with severe mental health disabilities. Responsibilities include obtaining prior authorizations for medications and treatments, enrolling clients in drug assistance programs, medical case management for medication clients, coordinating follow-up on no-shows, returning calls, triaging client concerns, obtaining vital signs, following up on requested labs, ensuring medical record completeness, reviewing charts for relevant information, and providing backup support for nursing staff. Requires strong computer skills and experience with the mental health population. Monday–Friday, 8:00am–4:00pm, located in Toledo, OH.
Required Qualifications
- Licensed Practical Nurse (LPN) license
- High computer proficiency
- Experience working with the mental health population
