Provides direct patient care in hospice, implements current nursing practice per plan of care, documents services, educates patient and family, coordinates hospice services with interdisciplinary team, and manages pain/symptom control. Must hold an active LPN license in-state, CPR certified, and driver with insured vehicle; prior home health or hospice experience preferred.
