Job Summary
LPN for full-time Home Health role in Shelby County, TN. Provide direct patient care in patients’ homes, carry out care plans developed by the RN, educate patients and families on self-care, document care accurately, and coordinate with the RN and care team to ensure comprehensive home-based support. Works with an interdisciplinary team and adheres to infection-control techniques; schedule is Monday–Friday, 8:00 AM–4:30 PM, with mileage reimbursement and comprehensive benefits.
Required Qualifications
- Active LPN/LVN license in the state
- Graduate of an accredited practical/vocational nursing program
- 3+ years of nursing experience (Home Health, Med-Surg, or Community Health preferred)
- Current CPR certification
- Reliable transportation, valid driver’s license, and auto insurance
