LPN

Provide skilled nursing care directly in patients' homes while administering treatments, medications, and monitoring condition changes. Collaborate with the Registered Nurse, physicians, and interdisciplinary team to complete visits, educate families on self-care, and maintain accurate clinical documentation. Adhere to established plans of care, infection-control protocols, and safety procedures to support patients in managing chronic conditions and maintaining independence. Full-time Monday–Friday schedule with competitive hourly pay, mileage reimbursement, and paid time off.
