Job Summary
Assess individual health care needs and assist in creating problem/needs lists with the nursing administrative team. Administer medications as assigned, write daily shift progress notes and weekly nursing assessments, and notify the Health Care Provider of any medical changes. Physically assist residents with transfers, bathing, and personal hygiene tasks, while educating staff and individuals on meal preparation and nutrition. Follow plans of care for each person in the residence, communicate discharge instructions and treatment changes to the Residential House Manager, and advocate for the rights of all individuals served. This role supports a small group of 2-5 individuals with disabilities in a residential home setting in Leicester, MA, with a bi-monthly review of nursing lists and on-call availability for hospitalizations.
Required Qualifications
- Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing
- At least one year of full time working experience as an LPN
- 21+ years old
- Valid driver's license
- Daily use of a car on site
- Available by telephone for daily contact
- Maintain CPR certification
- Maintain Safety Care certification, if required by position/program
- Complete Agency Nursing Orientation
- Complete MAP training audit
