Job Summary
Assess individual health care needs, create problem lists, and administer medications as assigned. Physically assist residents with transfers, bathing, and hygiene tasks while educating staff on meal preparation and dietary requirements. Document daily progress notes, communicate medical changes to providers and the House Manager, and advocate for resident safety and rights in compliance with agency policies. This role involves providing direct care to a small group of individuals with disabilities in a residential home setting in Leicester, MA. The position requires a current Massachusetts LPN license, one year of full-time experience, and availability for a Sunday and Monday shift from 7 a.m. to 3 p.m.
Required Qualifications
- Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing
- At least one year of full time working experience as an LPN
- 21+ years old
- Valid driver's license
- Daily use of a car on site
- Available by telephone for daily contact
- Maintain CPR certification
- Maintain Safety Care certification, if required by position/program
- Complete Agency Nursing Orientation
- Complete MAP training audit
