Job Summary
LPN position in Hospice serving Farmington, ME and surrounding areas. Provide in-home and community-based nursing per plan of care under the supervision of the RN, educate patients and families on preventive care and decisions, promote patient independence, perform nursing processes in care delivery, assess home/family safety, and participate in community health activities. Requires 2-3 years of acute care nursing or equivalent, licensure in Maine, CPR, and reliable transportation. Flexible scheduling with benefits including education reimbursement and PTO; opportunity for internal promotions.
Required Qualifications
- Graduate of an approved Practical Nursing program
- Current LPN license in the State of Maine
- CPR Certification
- Reliable transportation with valid Maine driver’s license
