Job Summary
Direct interventions in the community within a low-threshold harm-reduction model for people with drug and alcohol issues. Provide professional, pragmatic, holistic assessment, care planning and structured interventions across the Northern Trust area. Manage a fluid caseload, engage with service users through assertive outreach, and liaise with statutory, voluntary and community services to ensure high-quality services. Work independently with strong risk-management skills, and contribute to innovative service delivery methods while supporting housing, finances, health and family relationships. Includes comprehensive induction and training and a commitment to equality of opportunity.
