Low Acuity Home Health Nurse (RN or LPN)

Nurse (RN or LPN) needed for in-home patient care in NM on a 1099 contract basis. Responsibilities include executing physician-prescribed plans of care, documenting care in the system, administering prescribed medications and therapies, conducting patient assessments, coordinating care, assisting with daily living activities, monitoring vitals, wound care, and skin integrity, communicating with family, physicians, and case managers, and providing education to patients and families. Work is performed one-on-one in patients' homes, collaborating with CNAs/HHAs and other care team members. Schedule is 4 hours per month; background check is required. The role emphasizes compassionate, patient-centered care and building lasting caregiver-patient relationships.
