Job Summary

Execute physician-prescribed plans of care and conduct patient assessments for low acuity home health services. Administer medications, monitor vitals and GI intake/output, and manage wound care while performing daily living activities. Complete compliant documentation in the system of record and collaborate with CNAs, case managers, and physicians to coordinate care. Provide education to patients and families, communicate health updates, and supervise personal care attendants. Work one-on-one in the patient's residence to ensure a comfortable environment, with care provided at the patient's home in Jarales, NM.

Required Qualifications

  • Associate or Baccalaureate School of Nursing
  • Current State or Compact License as a Registered Nurse or Licensed Practical Nurse
  • Current CPR certification
  • TB test and physical examination
  • Maintain required insurance
  • Good interpersonal skills
  • Hand-eye coordination and manual dexterity
  • Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients
  • Ability to operate under stressful conditions and make quick decisions
  • Dependable transportation
  • Willingness to commute to the patient's home
  • Ability to provide basic medical management up to medically fragile end of life care
  • 2 hours of WEEKLY care
  • Monday and Friday Mid Afternoon (2:00pm preferred)
