Job Summary
Qualified Independently Licensed Therapists (LCSW, LMFT, LPC) to join a geriatric SNF/LTC therapy team in Colorado. Responsibilities include completing diagnostic evaluations, psychotherapy, treatment planning, and in-person consultations at partner SNF/LTC facilities; collaborating with BHS psychiatrists, psychologists, care coordinators, and facility staff; documenting in the EMR (Athena I) in a timely manner; complying with agency policies and procedures. Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree
- Colorado licensure as an LCSW, LMFT, or LPC
- NPI (National Provider Identifier)
- Credentialed with Medicaid/Medicare a PLUS
- TB Test
- CPR/AED/First Aid Certification
- Basic computer knowledge
- Fluency in Spanish is a plus
