Job Summary
Provide direct nursing care, administer medications and treatments, and monitor residents for changes in physical, cognitive, emotional, and behavioral conditions. Obtain vital signs, report significant findings to RNs or physicians, and assist with admissions, transfers, and discharges. Supervise and delegate tasks to CNAs while collaborating with interdisciplinary team members to coordinate resident care. Maintain accurate EMR documentation, follow infection prevention protocols, and educate residents and families on care needs. Respond promptly to urgent resident needs and emergency situations while ensuring compliance with facility policies and New Mexico regulations.
Required Qualifications
- Graduate of an approved practical nursing program
- Current, active Licensed Practical Nurse (LPN) license in good standing in the State of New Mexico
- Current CPR or Basic Life Support (BLS) certification
- Strong clinical observation and resident assessment skills within the LPN scope of practice
- Ability to recognize and appropriately communicate changes in resident condition
- Strong organizational and time-management abilities
- Excellent communication and interpersonal skills
- Ability to provide direction and support to nursing assistants and work collaboratively with licensed nursing personnel
- Ability to manage multiple responsibilities in a fast-paced clinical environment
- Compassionate and professional approach to caring for older adults and individuals with complex healthcare needs
- Commitment to resident safety, quality care, regulatory compliance, and positive clinical outcomes
- Ability to assist with lifting, transferring, repositioning, and mobility needs of residents using appropriate assistance and equipment
- Ability to stand, walk, bend, reach, and perform nursing duties for extended periods
- Ability to conduct resident rounds and provide direct nursing care throughout the facility
- Ability to safely operate applicable clinical and resident-care equipment
- Ability to respond promptly and effectively to changes in resident condition and emergency situations
- Ability to perform the essential functions of the position with or without reasonable accommodation

- Previous skilled nursing, long-term care, rehabilitation, or healthcare experience
