Job Summary
The Long Term Care Case Manager is responsible for planning and implementing VNH Long Term Care Service programs, managing a caseload of Medicaid clients, and ensuring access to resources for home living. Responsibilities include providing case management, resolving client and family issues, supervising care provided by VNH PCAs, and monitoring various databases for the OneCare program. The role requires collaboration with community partners and staff across VNH programs, alongside competencies in problem-solving, planning, and quality advocacy.
Required Qualifications
- Bachelor’s degree in social work or related field or LPN/RN
- Two years of experience in health care or related field
