Job Summary
Provide direct patient care through office visits, telehealth, and same-day appointments in Medford clinic locations across Northern Oregon. Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions while developing personalized treatment plans including lifestyle interventions and prescriptions. Educate patients on preventive health principles and collaborate with internal health coaches, nutritionists, and mental health providers. Document visits in the NextGen EHR and participate in team meetings and quality improvement initiatives. This flexible locum assignment offers full-time or part-time options for three months with extension possibilities.
Required Qualifications
- Active Oregon license as a Nurse Practitioner, PA-C, MD, or DO (or eligibility to obtain)
- Board certification in Family Medicine, Internal Medicine, or applicable specialty
- Minimum 2 years of clinical experience
- Strong interest in preventive, functional, and lifestyle medicine
- Excellent communication and patient relationship skills
- Comfortable working in a fast-paced, collaborative environment
- Proficient in using electronic health records (NextGen)
Desired Qualifications

- FNP - Family Nurse Practitioner license
- AGNP - Adult Gerontological Nurse Practitioner license
- AGNP - PC - Adult Gerontological Nurse Practitioner license
