Job Summary
Obtains and records vital signs and weights per policy while providing hands-on patient care, including grooming, toileting, incontinence care, and feeding. Implements care plans, performs skin care treatments, and uses standard and specific precautions for all patients. Assists physicians and nurses with treatments, collects soiled linen, and provides emotional support. Reports changes in patient conditions or concerns to the charge nurse or supervisor. Collaborates with other departments to ensure timely, effective care consistent with individual preferences. Requires an active NH LNA license, TB shot, physical exam within the last year, and reliable transportation with valid vehicle insurance. Must be licensed or license eligible in VT and NH.
