Job Summary
Oversee and supervise street outreach teams, including Housing Advocates and social workers, while providing structured supervision and implementing accountability systems. Manage urgent requests, lead the team during Code Blue/Red Alerts, and assess involuntary removals to ensure accurate DHS documentation. Promote effective outreach strategies using Housing First, Harm Reduction, and trauma-informed care models while developing relationships with external partners like NYPD and EMS. Provide on-call coverage as needed and attend onsite meetings with community providers. This role supports a program connecting over 500 individuals annually to housing and essential services, ensuring high retention rates through individually tailored care.
Required Qualifications
- NYS Licensed Masters Social Worker (LMSW)
- Previous supervisory experience
- Able to obtain 9.58 credentialing within first 3 months of hire
- Understanding of Housing First
- Understanding of client-centered motivational interviewing
- Understanding of Harm reduction models
- Understanding of behavioral and medical health systems
- Experience with crisis intervention
- Experience with risk assessments
- Experience with incident management
- Detail oriented
- Ability to manage team tasks
- Valid driver's license from NY, NJ, CT or PA with good driving record
- MS Office Suite proficiency
