Job Summary
Conduct intake assessments to evaluate Member readiness and determine appropriate level of care. Facilitate group and one-on-one counseling sessions using Affect's evidence-based curriculum to support recovery and promote wellness. Provide crisis support and coordinate appropriate referrals when needed, while maintaining timely, accurate documentation in the electronic medical record system. Collaborate with clinical supervisors on individualized treatment and discharge planning, ensuring compliance with HIPAA and all relevant state regulations. Applicants must hold an active, verifiable Illinois license (CADC, CRADC, LCPC, or LCSW) with credentials confirmed via the State Board's website. This full-time role offers a salary of $55,000 - $70,000 commensurate with experience and includes medical, dental, vision, and 401k benefits.
Required Qualifications
- Active and verifiable license from one of the following Illinois licenses: CADC, CRADC, LCPC, or LCSW
- Proficient with Microsoft Office or Google Suite
- Proficient with EMR systems
- Proficient with telehealth platforms
- Have a private, HIPAA-compliant workspace
- Have reliable internet
