Job Summary
Conduct IDT meetings with nursing facility staff and BHS clinicians to coordinate care. Audit patient charts to ensure timely and accurate behavioral documentation. Summarize patient behavioral records for submission to special needs programs and update tracking reports for approved and pending participants. Manage administrative duties related to processing and tracking patients on special needs programs. This role requires travel through the Lincoln area, with mileage reimbursement provided. Candidates must have prior experience in a Skilled Nursing Facility or Behavioral Health Unit.
Required Qualifications
- COVID-19 Vaccination and Booster
- High School Diploma or Equivalent
- Associate's or bachelor's degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field from an accredited college or university
- Two years of experience working in a behavioral health setting
- Moderate computer skills
- Ability to communicate effectively with clients, their loved ones and other professionals
- Ability to make sound decisions in emergency situations
- Ability to write reports and clinical documentation
- Travel through the Lincoln area
- Full-Time, W2 Employment (Monday - Friday)
Desired Qualifications
- Experience working in a Skilled Nursing Facility (SNF) or Behavioral Health Unit
- Non-violent Crisis Prevention Training
- CPI Certification
- Experience with electronic medical records
