Job Summary
The Integrated Health Care Clinician provides clinical and case management services across the lifespan within an integrated health care setting combining primary care, behavioral health, and dental services. The role works as part of a multidisciplinary team to conduct screening and evaluation, formulate DSM-based diagnoses, develop individualized and measurable treatment plans, and provide or refer for psychotherapy (individual, group, family) while coordinating with clients, families, and community resources. Responsibilities include triaging clinical emergencies, facilitating referrals, documenting in accordance with policy, supporting licensure goals, and contributing to the program’s goals and outcomes. Qualifications emphasize a Master’s degree (or doctorate in psychology) and licensure or licensure-eligible status (LPCA, LMFTA, LPC, LMFT, Psychologist; LMSW/LCSW as applicable) with a focus on behavioral health and integrated care; NHSC loan repayment and sign-on bonus are available. Required Qualifications
- Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field OR doctoral degree in Psychology
- Licensure Required: LPCA, LMFTA, LPC, LMFT or Psychologist; MSW/LMSW or LCSW as applicable to begin employment
