Job Summary
LifeSharing Specialist responsible for all case management related to the LifeSharing through Family Living program, screening and approving prospective providers, developing and monitoring Individualized Service Plans, ensuring regulatory compliance, coordinating training and respite care, and advocating for individuals served. This full-time role involves liaising with families and contracted providers, monitoring placements, and performing related duties to support individuals with disabilities in community settings. Requires a Bachelor’s degree in a related field (or equivalent work experience), strong communication and organizational skills, leadership ability, and a valid PA driver’s license. Duties span coordination with the Director of Family Support Services, provider-home monitoring, and ensuring program quality and compliance across multiple locations in Berks County.
Required Qualifications
- Bachelor's Degree in Special Education, Education, Psychology, or related field; or high school diploma and extensive experience working with persons with mental retardation.
- Ability to interact with and relate to persons with mental retardation.
- Demonstrated leadership abilities.
- Experience with designing and implementing outcomes.
- Experience with skill assessment instruments.
- Excellent verbal and written communication skills.
- Strong organizational ability and the ability to work independently.
- Ability to represent ProBerco in a professional manner in all settings.
- Possess a valid PA driver’s license.
