Job Summary
Teach skills to promote independence, mentor individuals to build social skills, and provide guidance on daily activities while assessing gaps between current and desired life states. Oversee and direct individual activities, assign work placements, and ensure Person-Centered Plan goals are implemented. Provide transportation, monitor safety, and document activities in electronic health records. Establish relationships with agencies, parents, and employers to encourage natural supports and leisure participation. Submit payroll data and complete required training and certifications.
Required Qualifications
- Must be at least 18 years of age
- Pass criminal record and healthcare registry check
- Pass driving record check if providing transportation
- High school graduate or equivalent (GED)
- Commitment to achieving competency in required knowledge, skills, and abilities
- Current First Aid/CPR blended or classroom certification
- Ability to lift up to 40 pounds independently
