Job Summary
Licensed Practical Nurse (LPN) at Holton Manor in Elkhorn, WI, on-site. Provide safe, effective nursing care per care plans and physician orders; administer medications and treatments accurately; assess residents and report changes promptly; document care in the EMR; monitor performance and mentor CNAs; ensure infection control and safety compliance; complete all other duties as assigned. Will work within a 60-bed skilled nursing community offering full-time, part-time, or PRN roles with flexible scheduling options (days, evenings, nights, weekends) and shift differentials. Qualifications include an active Wisconsin LPN license (or licensure by reciprocity from another U.S. state in good standing), CPR certification, strong English communication, and the ability to meet physical requirements. Preferred: long-term care or skilled nursing experience and clinical experience.
Required Qualifications
- Active LPN license in state of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity. (in good standing)
- CPR certified
- Ability to meet physical and job-related requirements
- Must be English literate and possess excellent written and verbal communication skills
