Job Summary
Licensed Vocational / Practical Nurse (LVN/LPN) needed for home health at Elara Caring. Provide one-on-one patient care under the direction of an RN and physician, follow the Plan of Care, monitor patient conditions, educate patients and caregivers on disease management and self-care, document assessments and interventions, and collaborate with the care team to ensure quality outcomes. Must be a graduate of an accredited Associate Degree Nursing program with an active LVN/LPN license in good standing and at least one year of clinical nursing experience (Home Health/Hospice preferred). Ability to lift up to 50 pounds and travel locally (~50%) required. Location: Manchester, NH, United States. Required Qualifications
- Graduate of an accredited Associate Degree Nursing program
- Current state LVN/LPN license in good standing
- Minimum of one (1) year of clinical nursing experience
- Home Health or Hospice experience preferred
- Strong clinical assessment and communication skills
- Ability to lift, push, pull, and support up to 50 pounds as needed
- Valid driver's license, reliable transportation, and current auto insurance
- Willingness to travel locally (approximately 50%)
