Job Summary
Provides basic nursing care by assisting in the assessment of the resident on admission and on an ongoing basis; communicates changes to the IDT and residents/families; documents assessments; participates in care planning and updates care plans; implements nursing interventions including medication administration; ensures safe and timely interventions; administers IV fluids if IV certified; demonstrates critical thinking and calmness during crises; maintains SBARR communications with physicians; monitors progress and uses QAPI processes for problem resolution; engages in ongoing nursing education and self-improvement; role involves collaborating with interdisciplinary team and supporting residents’ educational and environmental needs.
Required Qualifications
- Education from an accredited school of nursing resulting in becoming a Licensed Practical Nurse
- Current license in the state you will be working as a Licensed Practical Nurse (LPN)
- IV Certification required or willing to obtain within six months of employment
- Current CPR/AED Certification required or willing to obtain within six months of employment
- Must be able to assess the basic nursing care needs of geriatric residents
- Must possess excellent communication skills to interact with residents, visitors and staff
- Excellent time management and organizational skills are required
- Must possess critical thinking skills
- May be assigned other duties as appropriate
- Long-term care/Rehab/Assisted Living experience preferred
