Job Summary
Provide nursing services, medication administration, and supervision of staff within a community-based setting for individuals with brain injury, IDD, substance use disorder, or behavioral disabilities. Conduct assessments, implement health care services per ISP, train staff on universal precautions, infection control, and medication administration, and coordinate with medical providers to ensure consistent treatment. Role supports varied schedules (full-time, part-time, as needed) in California and requires nursing licensure (LPN/LVN) and an associate’s degree; responsibilities include patient monitoring, documentation, education, advocacy, and collaboration with external agencies.
Required Qualifications
- Associate’s Degree in Nursing or related field, or state certificate
- Current state LPN or LVN licensure for state of residence
- One year of experience working with population served in a nursing capacity

- CPR/FA certification
- HIV/AIDS certification
- BBP
- ACT training
