Job Summary

Record patient information, medications, and pre-visit summaries in the electronic medical record. Monitor patients for drug reactions, observe changes in condition, and promptly inform providers of urgent situations. Administer medications and vaccines orally, rectally, or by injection while following standing orders and protocols. Prepare exam rooms, assist with patient preparation, and perform laboratory testing including routine waived tests and quality control monitoring. Conduct screenings using glucometers and spirometers, verify discharge instructions, and refer patients to social services or pharmacy as needed. Maintain adequate stock levels, document action logs, and ensure compliance with PCMH principles and safety policies. Work evenings, weekends, and travel to clinic sites as required while maintaining bilingual communication in English and Spanish.

Required Qualifications

Licensed vocational nurse in the State of Texas
Must be current with BLS/CPR
Bilingual (English/Spanish)
Current valid Texas driver's license for the appropriate type of vehicle
Texas Liability insurance
High School or GED
Must be able to lift 20 lbs
Ability to bend, stretch, stoop, stand, and sit
Ability to speak, understand and write Spanish and English
Must be able to apply critical thinking skills, multi-tasking, prioritization and attention to detail
Ability to interpret and translate for the provider when necessary
Ability to initiate prompt and accurate treatment, diagnostic screening and medications as ordered
Ability to recognize, evaluate, solve problems, correct errors, and report to supervisors of issues
Ability to resolve patient service issues and exercise independent judgment

Ability to read, write and speak in a clear, accurate and professional manner

Knowledge of SC policies and procedures
Knowledge of the Chronic Care Model and Patient-Centered Medical Home Model (PCMH)
Skilled in excellent customer service
Ability to complete basic/intermediate math computations and medical math conversions
