Job Summary
Conduct beneficiary health screenings, medication reconciliation, and pill counts during intake. Oversee client self-administration of medications, coordinate refills, and resolve prescription issues. Participate in multidisciplinary team meetings and case conferences to provide primary care, obstetric, mental health, and MAT appointment management. Upload medical documentation to the electronic health record system and notify the medical director for review. Manage withdrawal scales for detox clients, triage medical needs, and oversee TB testing for clients and staff. Maintain current BLS certification, California LVN status, and CPR/First Aid training while working independently under the medical director's supervision in a fast-paced, client-centered environment.
Required Qualifications
- Minimum 1-year experience as a licensed vocational nurse
- experience treating clients with chemical dependency
- experience treating clients with mental health
- experience treating clients with co-occurring disorders
- current BLS certification
- active California LVN status
- Ability to work independently under the auspices of the medical director
- discharge duties within the licensure and scope of a licensed vocational nurse
